If the problem only happens under braking, then the problem has to be in the brakes. When you replaced the front pads, did you do anything with the rotors? If not, then you may need to replace them or have them machined. Personally speaking, I’d much rather just replace rotors than machine them. Also, make sure that you tighten the wheels down properly, meaning a star-pattern on the lugs and a torque wrench. If memory serves Dodge calls for 100 ft-lbs of torque on the lugnuts on your van.
