My first thought is that you may have the A/C or Defrost turned on. The engine idle is kicked up anytime the A/C compressor clutch is engaged to compensate for the additional load on the engine. A/C is turned on anytime the defrost setting is used to aid in defrosting the windshield and keep the A/C seals in good condition during the winter.

I would take a look at your IAC.
You could try cleaning it.
check the iac valve. But if you do decide to clean it DO NOT touch or move the metal spring in anyway it will damage the valve.
I did clean the iav valve with cab cleaner. not sure if i damaged it. Im trying to get my car to pass CA emissions the car tested high on HC the limit is 55 my car measured 84. I guess it failed beause of the high idle not sure. I also cleaned the EGR valve in an attempt to fix this
I am experiencing the same problem with 99 Maxima 5 speed. The idle would shot up to 1500, but in most times around 1000. Cieaned the ICV but it did not help and only if engine is cold. When engine warms up it idles at 500. No CEL.
I think I am dealing with some type of intake air leak, but not sure where. Note that ICV has gasket at the mounting point. Every time you open you should replace it.
the idle was perfect before i replaced the plugs with NGK then clean the IVC. does the whole throttle body need to be replaced?
I haven’t had much luck cleaning IAC valves, you might consider replacing it. The fact that it idles fine sometimes and not others would lead me to suspect something with the IAC rather than a vacuum leak or other mechanical issue. If that was the case the problem would likely be there all the time.
